New York City Primary Care Center Emergency Preparedness Assessment 2004
The Primary Care Center Emergency Preparedness Assessment Tool, developed in May 2004, evaluates primary care centers emergency management and preparedness. The tool is divided into five sections: Center Experiences; General Preparedness; Education and Training; Coordination and Response; and Operations and Logistics.

Primary Care Emergency Preparedness Matrix
Utilizing the assessment tool discussed above, the Primary Care Emergency Preparedness Matrix categorizes primary care centers into one of five levels of emergency preparedness, across 14 categories of preparedness.

Primary Care Emergency Preparedness Integration Project
BHPP works closely with the Primary Care Development Corporation (PCDC) on projects to implement emergency preparedness measures in Article 28 primary care centers across New York City. This project uses planning tools and templates developed by PCDC, DOHMH, and the California Primary Care Association. Finalized documents will be posted when they are available.
