HIV Care, Treatment, and Housing

This site offers information and resources for HIV-related health care, housing, and supportive service providers.  It contains information to assist providers with referrals for clinical care, medical case management, housing, and other supportive services funded by the Ryan White Part A Program for uninsured and underinsured individuals living with HIV in New York City.  Providers can also find housing assistance and supportive services funded by the Housing Opportunities for Persons with AIDS (HOPWA) program for low-income HIV-infected persons and their families.  The site also provides key documents on the New York City Department of Health & Mental Hygiene’s Care Coordination program.
