LL4 - Local Law 26/04 Affidavit of Acknowledgment of Sprinkler Requirements
Effective 10/22/04, Local Law 26 of 2004 ("LL 26/04") amends Local Law 5/73. The new code provisions require owners of all buildings that are one hundred feet or more in height that have not already installed sprinklers throughout their building to file a one-year report containing an affidavit by July 1, 2005. An owner may still file for either a hardship extension pursuant to BC § 27-228.5(b) or a waiver pursuant to BC § 27-929.1(2) following submission of this affidavit. However failure to submit this affidavit by July 1, 2005 may jeopardize the ability of the owner (or the owner's successor) to claim a hardship extension pursuant to subdivision (a) of BC § 27-929.1. Please submit this affidavit to the Department of Buildings, Local Law Enforcement Unit, 280 Broadway - Room 401, New York 10007.

LL11 - Local Law 11/98 Intake/Transmittal Initial/Amended Report Form
This form is used as a receipt and retrieval of Local Law 11/98 technical reports. The submission of all Local Law 11/98 technical reports, must be accompanied by the appropriate LL 12 and LL 11 forms listing all the addresses of the buildings being submitted to DOB. The form is date stamped received and returned to the respondent. It must be presented at the counter when retrieving a submitted report.

LL12 - Local Law 11/98 Intake/Transmittal Initial Report Unsafe
This form is used as a receipt and retrieval of Local Law 11/98 technical reports. The submission of all Local Law 11/98 technical reports, must be accompanied by the appropriate LL 12 and LL 11 forms listing all the addresses of the buildings being submitted to DOB. LL 16/84 - Report of Compliance with Local Law 16/84
Local Law 16/84 is a fire safety code. The requirements to comply vary depending on the occupancy. The law requires building owners to notify the Department of the Buildings regarding the compliance status of their building by completing this form. The owner must specifically indicate on this form what work has been completed or partially completed to comply with the law. This form must be notarized. Duplicate copies are to be submitted to the Department of Buildings.

LL 26/04-1 - Photoluminescent Affidavit/Report for Existing Buildings
Use this form to certify compliance with the photoluminescent exit path marking requirements of LL 26 of 2004, 27-383(b), and RS 6-1, for high rise office buildings constructed pursuant to plans filed before July 1, 2006. This form may be completed only by an architect, engineer, the legal owner of record, or the owner’s managing agent.

LL 26/04-2 - Photoluminescent Reconsideration Application
Use this form to request a reconsideration (variance of technical requirements) of compliance with the photoluminescent exit path marking requirements of LL 26 of 2004, 27-383(b), and RS 6-1. This form must be signed by an architect, engineer, the legal owner of record, or the owner’s managing agent. In addition to the required signature, a contractor or installer may also sign in cases where such contactor or installer will be the person attending the reconsideration appointment.

LL 26/04-3 - Local Law 26/04 - Report of Compliance Power Source for Exit Signs
LL 26/04 (Fire Safety) requires hi-rise buildings in occupancy groups C, D & H and all buildings in occupancy groups E, G & J-1 to have all exit and/or directional signs in the the building connected to an emergency power source or storage battery equipment. The professional must submit this form to the Deaprtment of Buildings certifying that the building is in compliance.
