Elevator Appointment Online Request Form Upload Instruction

Elevator Application Upload Instructions
Must contain one of the following documents:
Elevator Application (EA), Elevator Building Notice (EBN), Amendment (B Form 10) or Punch List.
  1. After receiving approval from Plan Examiner, scan and save the elevator document to your local hard drive.

  2. Go back to the appointment online request form and attach the elevator document located in your computer.

  3. Upload complete


Payment Invoice Upload Instructions
Must contain one of the following documents:
Expedited (Pre-Clearance), Missed Appointment or Unprepared Appointment.

  1. If attaching a payment invoice, the elevator application must be uploaded or request will be rejected.

  2. Payment document must be an invoice by the 4th floor cashier for the requested application.

  3. Scan and save the invoice document to your local hard drive.

  4. Go back to the appointment online request form and attach the invoice document located in your computer.

  5. Upload complete.
