To renew your Welder License, submit the following documents by mail to:

• Original, typed LIC2 License Application available on the Department website. All fields of the LIC2 application in sections 1,2,3,4,6,9 & 10 must be fully completed or your application will be returned to you.

• Completed, typewritten LIC34 Licensing Supplemental Affidavit (if applicable)

• A copy of your AWS or NYSDOT Welder Certification

  • The American Welding Society (AWS) Certified Welder certification must be for the Manual-Shielded Metal Arc Welding Process (SMAW), Groove and fillet welds, 3G & 4G (all) positions, unlimited thicknesses (1” plate), E7018 Filler, under D1.1 Code.   • The New York State Department of Transportation (NYSDOT) Field Welder Certification must be for the Manual-Shielded Metal Arc Welding Process (SMAW), Groove and fillet welds, 3G & 4G (all) positions, and unlimited thicknesses (1” plate). For information on how to schedule a certification test with NYSDOT please visit: www.nysdot.gov

• $45 fee payment (check or money order only); include your address on the check

• One 2x2 current photograph (passport size)

• Your New York City Welder License card

• LIC60 Visual Acuity Form for Welders

• Child Support Certification Form required for all licensees

NOTE: To avoid an additional late fee of $50, renewal applications must be submitted 30-60 days prior to the expiration date indicated on the license card. If renewal is received less than 30 days prior to expiration the total fee will be $95.
