• Original, typewritten LIC2 License Application

• Completed, typewritten LIC34 License Affidavit (if applicable)

• One 2x2 current photograph (passport size)

• Copy of Site Safety Manager Refresher Course Certificate (course must be taken within one year of renewal)

• $150 fee payment (check or money order only). If paying by check, make sure to include your address.

To avoid a late fee, renewal applications must be submitted 30-60 days before the expiration date indicated on the license card.
