Original, typewritten LIC2 License Application
Copy of Site Safety Manager refresher course completion certificate (course must have been taken within two years prior to renewal)
One 2x2 current photograph (passport size)
$50 fee payment (check or money order only). If paying by check, make sure to include your address.
To avoid a late fee, renewal applications must be submitted 30-60 days before the expiration date indicated on the license card.
