To renew your Class A or B Hoisting Machine Operators license you must submit the following to the Department's Licensing Unit:

• A completed and typewritten LIC2 a application

• A completed and typewritten LIC34 License Affidavit (if applicable)

• Completed Buildings Physical Examination Form (LIC61) signed by a physician or a current signed Department of Transportation Medical Examiner's Certificate

• A notarized letter that lists the name, address and phone number of the laboratory that performed your substance abuse test and consent to release results to the Buildings Department upon request

• A copy of a valid photo ID of yourself (driver license, passport or green card with photo)

• One 2x2 current photograph of yourself (passport size)

• Your $150 license renewal fee (check or money order only). If paying by check, make sure to include your address

Once gathered, all the documents should be mailed to the Licensing Unit at:

Once your renewal application and supporting documents are approved, you will be contacted to schedule an appointment with the Licensing Unit to come in person and exchange your license card for your new card.
