Hoisting Machine Operator Class A & B License Renewal

To renew your Hoisting Machine Operator Class A or B license, you submit the following documents in-person between 9am – 12:45pm to:

  • Original, typewritten LIC2 License Application;

  • Completed, typewritten LIC34 Licensing Supplemental Affidavit (if applicable);

  • One 2x2 current photograph (passport size);

  • DOB Physical Examination Form Buildings Physical Examination Form (LIC61) signed by a physician or a current signed Department of Transportation Medical Examiner’s Certificate;

  • Notarized letter from the Licensee that includes the physician’s/laboratory’s name, address, phone number and your consent to release the results of the substance abuse test to the Department upon request;

  • Current NCCCO or CIC Certification card;

  • Eight (8) hour refresher course certificate;

  • Child Support Certification Form;

  • Driver’s License, or valid government issued ID;

  • Current/Expired NYC Hoisting Machine License card;

  • $150 renewal fee.

To avoid a late fee, renewal applications must be submitted 30-60 days before the expiration date indicated on the license card.

NOTE: The applicant must have the current NCCCO or CIC Certification(s) in order to renew. See the National Crane Certification Guide for information about which certifications are required.

Beginning July 1, 2015, during the one (1) year immediately prior to renewal, the licensee must have successfully completed a Department-approved eight- (8) hour course. For the HMO refresher course registration go to:
