- If an assist crane is needed to assemble the mobile crane (but not to raise the completed tower/boom/jib of the crane under assembly), an amendment must be filed to cover the assist crane. Submit three (3) copies of CD form 10 (Rev. 12/03), foundation calculations, location of the crane and foundation sketch. All materials must be signed and sealed by a Professional Engineer or a Registered Architect.
- If the inspection is to be performed by Hoist and Rigging inspectors in other than normal working hours, submit three (3) copies of After-Hour Inspection request form (CD-21 Rev. 2/05) plus the inspection fee of Seven Hundred and Fifty Dollars ($750.00) per day. Also submit five (5) copies of application form for After-Hour Work Permit (PW-5 Rev. 5/88).
- For cranes to be assembled off-hour under Directive 14/75 (inspection performed by applicant under controlled inspection), submit five (5) copies of application form for After-Hour Work Permit (PW-5, Rev. 5-88) and the approriate fee of One Hundred Dollar ($100.00) application fee plus Eighty Dollars ($80.00) per day permit fee. If crane is to do construction work a work variance application should be filed with and approved by the Borough Office.
- No load shall be swung over occupied building or pedestrian and vehicular traffic.
- All submitted forms shall be properly endorsed. Drawings calculations and relevant forms shall be signed and sealed by a Professional Engineer or a Registered Architect.
- If crane is located on the street or sidewalk, submit Street/Sidewalk Closing Permit from the Department of Transportation, Bureau of Highways; or Street/Sidewalk closing stipulation approved by M.T.C.C.C.
- If crane is located above or near the subway/train or structure, applicant should secure concurrent approval from Transit Authority or applicable agencies.
- For cranes over 250 feet in tower/boom/jib combined length, or for the use of derricks, magnetflux (magnetic particle inspection) report on the weldment of laced sections shall be available on site for use by Hoist and Rigging Inspector. Alternatively test report using other non-destructive methods (X-ray, ultrasonic, dye-penetrant etc. may be used if the certified testing laboratory or Professional Engineer determines that the procedure is applicable to the material and weldment of the unit under test.
DRAWING NOTE (I)
Submit Four (4) copies (minimum) of drawing which should include, but not be limited to, the following information:
Operation note warnings; plan showing the site and adjacent streets and properties; other cranes on site; height of existing adjacent buildings and building under construction; street width; crane location and orientation; trees, signs and obstacles; sidewalk sheds and barricades; traffic directions; pick and unloading points; revving information (# of parts, cable size etc.); roof protection; swing plan; and crane foundation design. Drawings should be submitted in sets of four (4).
DRAWING NOTE (II)
All information required under drawing note (I) plus: Design of tower crane/derrick foundation; bracings; tie-ins; guys and anchors; deflector sheaves; posting and shorings; fleet angles; weldment; padeyes and pins; erection/dismantling plan; elevation view and details; and for steel building, the landing of mateial plan.
MASTER RIGGER NOTE
For rigging work under the scope of a Master Rigger License, except for Cranes with combined tower/boom/jib length under 250 feet. submit all Engineering information required under “On Site Inspection by Hoist and Rigging Inspectors” except form CD-4, TR-1 and the application fee for On Site Inspection. In lieu of the CD-4 form, submit Site Safety Co-ordinator Letter (CD-12). Also submit a copy of weight certificate for loads being Picked. For other requirements, see general notes.
