The Bureau of Electrical Control will be using a revised application form that calls for new information to be provided as part of a licensed electrician's application for a certificate of electrical inspection. This Policy and Procedure Notice is designed to explain the significant changes. The parts referred to below correspond to the parts of the revised form, a copy of which is attached.
- Part A - Job Location - BIS Number
The applicant must provide the BIS number of any job filed with the Department that is related to the electrical job he or she is filing with BEC.
Part A - Job Dates - Temp Service Requested
This box is entirely revised. We have eliminated the applicant's option to specify "ready now" or "will notify" and have replaced it with the option to specify "yes" or "no". If "yes" is checked, the owner must sign Part F (II) of the application indicating that the intended meter use shown in Part E complies with §3018 of the Administrative Code. BEC will review the request for temporary service and will approve or deny the request.
- Part A - Work to be Done, #9 Other
Although not a new category, this is where an applicant must identify a Local Law 16/84 application by writing the words "Local Law 16/84." Additionally, such an applicant must check "E" "Emergency Lighting" under Part A - Special Certification/Approval Required.
- Part B - Licensee Statement
This is a new statement required of applicants attesting to the truth of the material contained in the application as well as affirming that the work will be performed under his or her supervision and in compliance with all applicable codes and regulations.
- Part C - Owner's Information
This block is entirely new. It is patterned after the owner's authorization provisions of the Department's PW1. The person or entity whose information is sought is the person or entity who is hiring the contractor and who will provide access to the premises at the time of inspection. It is someone who has the legal authority to engage the contractor, to have the work performed, and to provide access upon its completion. The person or entity whose information is provided in this block is the person or entity who must sign the application in Parts F (I) and F (II).
This block also asks for information as to whether the work filed for was started or filed by others. If the firm name and/or identity of the individual (s) is known, it should be noted.
- Part E - Number of Meters
The information requested in this section is all new and requires the applicant to indicate the total numbers of meters located at the premises upon completion of the work and to specify the number of existing, additional, and new meters involved. For example, a two-family home with two existing meters that has a third common space (PLP) meter installed would be reflected as a total of three meters, two existing and one additional. Likewise, the same two-family home replacing both existing meters only, would indicate two meters total, two existing. Meters can be categorized as new only at locations where there are no pre-existing meters.
- Part F(I) - Owner's Authorization
The owner authorization is a new requirement. This block reflects the owner's authorization to have the licensee file the application and undertake the work. It also reflects the owner's commitment to provide access for inspection upon the work's completion as a condition to the issuance of a final certificate of inspection or the continued energizing of the work through issuance of a temporary certificate.
- Part F(II) - Owner's Statement of Intended Meter Use
This statement reflects the owner's acknowledgement that the new, additional, or existing meters will be used in compliance with the meter-to-occupancy ratios specified in the Electrical Code.
