Case Management Services

Case management agencies funded by the Department for the Aging (DFTA) help seniors who need assistance managing activities of daily living access the services and resources they need. Services include:

  • A visit to your home to assess your needs and develop a care plan.
  • Arranging for you to receive supportive DFTA-funded services such as home-delivered meals and/or home care. Note: You cannot receive DFTA home care if you are eligible for the Medicaid Personal Care Program. Your case manager will help you access the services you need from Medicaid.
  • Referral for other community- or borough-based services.
  • Evaluation of your benefit/entitlement needs.
  • Advocacy with landlords, utility companies, benefit programs.
  • Counseling on long-term care issues.
  • Regular contact with you to ensure that the services you are receiving continue to meet your needs.

Case management services are free. DFTA case management agencies are located in every borough.

Other Case Management Resources

Case management is also available from non-profit providers that are not funded by DFTA and that have their own payment policies.  It is also available from geriatric case managers in private practice.

Friendly Visiting

The Department for the Aging in partnership with Citymeals on Wheels, is now offering a Friendly Visiting Program specifically geared to seniors who have difficulty leaving their homes on a consistent basis.

If you are a senior who is interested in making a new friend, consider participating in DFTA’s Friendly Visiting Program. Through this program you will be matched with a screened and trained volunteer who will visit you in your home on a regular basis. You and your new friend will have an opportunity to spend time sharing common interests, ideas, opinions and experiences. Friendly visiting is an opportunity to expand your social connections and enjoy a mutually-enriching friendship.
