The Modernization Transition Center contains all the necessary information and updates providers can use to easily guide them through the transitions for case management, home delivered meals, and senior centers.
The Transition Center includes all communiqués sent from DFTA to providers and clients and further connects the Department's community partners with the individuals responsible with their portion of the transition.
DFTA's goal is to provide customized support for its community partners while making the transition seamless for seniors.
Case Management Transition
On June 30, 2008 the three month case management transition period for providers ended. During that time DFTA was in constant communication with outgoing and incumbent providers, including numerous individualized meetings to assist with the process. In addition, DFTA notified all clients and corresponding programs (Home-delivered meals, Social Adult Day Services Program, and Homecare) of the ongoing transition.
