Case Management:
DFTA launched its modernization efforts with the redesign of case management services. The case management transition began on April 1, 2008 and continued through June 30, 2008. The three month transition occurred incrementally, with specialized attention and support from DFTA. This case management piece was integral, as it is the first opportunity that an at-risk homebound senior has to meet with a trained case worker who can link them to a host of entitlement programs and in-home social services. By assisting older New Yorkers to maintain their independence and residence, DFTA's case management program circumvents the need for costly nursing home or alternative care settings. Among other things, the revamped case management system:
- Provides senior citizens with user-friendly and
comprehensive information and assistance in applying for City, State, and
Federal human service benefit programs.
- Creates clear jurisdictional boundaries for each case
management provider.
- Culls the expertise of top-notch social services providers with a history of providing services to the aging population.
Home Delivered Meals Program:
After the successful transition of case management services, DFTA focused its efforts on its Home Delivered Meals Program (HDML). The HDML Request for Proposals (RFP) was released on May 1st, 2008, and DFTA awarded contracts in December, 2008. The benefits of this HDML modernization effort include:
- The creation of an infrastructure that will support a
growing senior population;
- Greater consumer choice and flexibility in meal type
and frequency of delivery;
- Ensures delivery of meals that meet Federal and City
dietary guidelines and that are appropriate for medical, dietary and cultural
restrictions; and
- Creates a closer marriage between case management providers and HDML contractors, ensuring the highest level of care for each client.
Congregate Programs for Older Adults:
With the December appointment of Commissioner Lilliam
Barrios-Paoli, a new collaborative, community-driven process for modernization
of senior centers will begin. On December 19, 2008 the RFP for Congregate
Programs for Older Adults was withdrawn.
