Modernizing Case Management
Case management is the gatekeeper for in-home services such as home delivered meals and home care. The new system will provide a comprehensive and holistic approach to allow older adults to continue to live at home and be engaged in their communities.
While the case management transition has concluded, DFTA continues to want to be of assistance in anyway possible. Community partners with questions or in need of assistance, please e-mail Eileen Mullarkey at cmtransition@aging.nyc.gov
