Renewal Registration For existing fossil fuel burning boilers and water heaters greater than or equal to 350,000 Btu/hr and less the 2.8 Million Btu/hr that need to be renewed.

Submit Inspection Request to Obtain a Certificate of Operation The request for original inspection must be submitted by a Licensed Professional (PE / LOBI).
Performance Test Worksheet This form must be submitted along with the ‘Inspection Request’ – form AR 365.
Bare Boiler Compliance Certification Submitted by a Professional Engineer to certify equipment not accepted by DEP will comply with all applicable regulations.
Burner Compliance Certification Submitted by a Professional Engineer to certify equipment not accepted by DEP will comply with all applicable regulations.
