Exam Application Forms

To apply for a civil service exam by mail: 
  • Refer to the "Required Forms" section on the Notice of Examination for the
    form(s) that you must fill out. 
               o View exams open this month 
  • Return all completed form(s) and the application fee, payable by money order only to DCAS Application Unit, 1 Centre Street, 14th floor, New York, NY 10007.

Exam Forms and Information:

  • Application For Examination (APP) (Required) - standard form required for all civil service examinations.
  • Police Officer - Special application forms for Open Competitive and Promotion Police Officer exams.
  • Education and Experience Test Paper (EETP) - (Read the Notice of Examination to determine if this document is required and which sections you need to complete)
  • Affirmation Form - required for education-and-experience exams when an applicant submits information online. Forms are provided as part of the online process.
  • Foreign Education Fact Sheet (FEFS) - list of acceptable foreign education evaluation services.
  • Special Circumstances - use this form to request alternate test dates for Religious Observance, Special Accommodations for persons with disabilities, Veterans' Credits, Parent and Sibling Legacy Credit, Change of Address.

General Examination Regulations for Civil Service Exams

Applications for all Civil Service examinations may be obtained in person only at
2 Lafayette St., Manhattan, Monday thru Friday, between the hours of 9AM-5PM.
