Health Literacy Fellowship 2009 Application for Host-Teachers


Thank you for your interest in the New York City Health Literacy Fellowship and welcome to the online application process. The Host-Teacher Application is a chance for teachers to apply to host a fellow for three days per week for eight weeks during the summer of 2009. You will need to complete the application form available here and follow the instructions below.

This application has five parts:

  5. Signatures

Instructions:
When you have successfully submitted your application, you will receive a confirmation email. If you do not receive this, you have not successfully submitted your application.

Applications are due on April 1, 2009. You will be notified of acceptance on April 10, 2009.

The 2009 Fellows-Teacher matching process will be collaborative. Teachers and fellows will meet at "Match-Day." They will have the opportunity to sit down, one-on-one and talk about classes, expectations, strengths, and weaknesses. Teachers will rank their top three fellow choices. Fellows will rank their top three teacher choices. The Mayor's Office of Adult Education will assign placements based on these rankings. Match Day is Friday, May 8th, 2009 from 6PM-9PM. Attendance is required.

Timeline:
April 1: Application Due
April 10: Notification of Acceptance
April 18: Orientation, 9:00AM-4:00PM, Attendance optional
May 8: Match Day, 6:00PM-9:00PM, Attendance required
June 22: First Day of Fellowship
August 21: Last Day of Fellowship

Preference will be given to teachers:

  1. From programs with ESL and ABE classes in session during the fellowship dates, June 22-August 21, 2009. While fellows will only be team-teaching one class, exposure to both ESL and ABE class types is important for their holistic training.
  2. Showing willingness to mentor and coach the fellow in clear & effective communication skills and teaching techniques;
  3. Demonstrating dedication to implementing a health literacy curriculum for the eight-week summer;
  4. Describing how the fellow will be involved in curriculum development and implementation.
  5. Exhibiting a commitment to including the fellow in program-wide activities such as staff development and student intake.
