Fellowship 2009 Application for Medical Students
Thank you for your interest in the New York City Health Literacy Fellowship. Welcome to the online application process. You will need to complete the application form available here and follow the instructions below.

Instructions:
When you have successfully submitted your application, you will receive a confirmation email. If you do not receive this, you have not successfully submitted your application.
The application deadline is March 25, 2009.
You will need two letters of recommendation to complete this application, one from an academic reference and one from a non-academic reference. Your references should use the PDF forms provided in Part Five below. Letters of recommendation must be received (if faxed or submitted via email) or post-marked (if submitted by mail) by the March 15 deadline.
Interviews will take place the first week of April. You will be notified of acceptance by April 10th, 2009.
Successful candidates will:
1. Exhibit a desire to learn about clear & effective communication between doctors and patients.
2. Describe how the health literacy fellowship will contribute to their growth and development as future physicians.
3. Demonstrate their ability to work well in situations outside their comfort zone and outside the orderly world of medical schools.
4. Be available to the program for the duration of the 8 week summer session and the four introductory seminars.
Timeline
March 25: Applications Due
March 30-April 3: Interviews
April 10: Notifications of Acceptance
April 18: Orientation, 9:00AM-4:00PM
June 22: First Day of Fellowship
August 21: Last Day of Fellowship
Fellows will participate in two evening seminars, from 6:00PM-9:00PM, in May. These seminars are introductions to the New York City adult education system. The dates for these four seminars are to be determined.
