Bridges to Health

Bridges to Health (B2H) is a Medicaid Waiver Program that provides supportive, wrap around and after care services to children, youth and adolescents, their birth, foster and adoptive parents and siblings in the home in their communities.

B2H consists of three waivers: B2H for children with Serious Emotional Disturbances, B2H for children with Developmental Disabilities, and B2H for Medically Fragile children. Referrals must be made directly to the B2H unit at ACS.
