Documents to be submitted as Self-Assessment Submission 

Your submission should be comprised of completed forms found in Appendices A-F, which can be downloaded from the links below.  Each Appendix is an easy-to-fill-out excel file. 
We recommend you
1. Download each form (Appendices A-F)
2. Save each form on your computer; revising the document name ("Save As") to include your program name
3. Complete each form (remembering to save each as you work and complete them)
4. Submit the completed forms by attaching them to an email and emailing them to the self assessment mailbox at: SAR@acs.nyc.gov

Please note:  Sections of each worksheet have been locked.  Information should only be entered in unlocked, available cells.  Embedded formulae will automatically calculate totals and averages.

APPENDIX A. Attestation  (Excel Document)
one completed form per AGENCY; signatures will be obtained during the review session

APPENDIX B. ACS/CCHS Environment Rating Scale (ERS) Program Profile  (Excel Document)
one completed form per PROGRAM SITE; make and complete additional copies, if more than 10 classrooms are rated

APPENDIX C. ACS/CCHS Family Child Care Environment Rating Scale (FCCERS) Network Profile  (Excel Document)
one completed form per FCC NETWORK; make and complete additional copies, if more than 20 homes are rated
