Medical Record - Physician

To request a copy of a medical record from a physician, call or write to the physician holding the record.  

If the physician does not respond to this request within a timely manner, you can file a complaint with the NYS Department, Office of Professional Medical Conduct for Physician. Complaints can be made by phone or mail to:

New York State Department of Health
Office of Professional Medical Conduct

Troy, NY 12180-2299

Or you can call the Office of Professional Medical Conduct for Physician Complaint line (800) 663-6114 and request a complaint package.

You can get assistance with getting records from physicians that have gone out of business.
