The City maintains a record of immunizations administered in New York City to children and adolescents through age 18. All records are retained on a permanent basis.

Immunization records are available for children born in New York City after 1996, and for adolescent immunizations after 2005. In addition, the City may have a record of an immunization given to a child, adolescent, or adult if the vaccine was received in a City clinic after 1988.
