Opt-Out of Diabetes Intervention Information

The City provides information about submitting an A1C Registry Do Not Contact Request. The Board of Health requires the reporting of hemoglobin A1C, a measure of blood sugar control over three months, by most clinical labs that report electronically to the New York City Department of Health and Mental Hygiene. You can request not to be contacted by the Health Department regarding your A1C levels. The Health Department also will not send your A1C information to your health provider.
