resources for Alzheimer's Disease, dementia, and long term care for seniors as
their needs and conditions change with age. Information includes care options
such as home care, adult day care, respite care, nursing homes and assisted
living facilities that provide care for individuals with dementia or Alzheimer's
and who cannot live independently.
If you request assistance, a social worker will contact you
to get more information about the senior's circumstances.
