Birth Certificates from Puerto Rico

Effective Monday, November 1, 2010, all Puerto Rican birth certificates issued on or before June 30, 2010 will no longer be valid. New York City residents who were born in Puerto Rico can still get a copy of the new birth certificate by mailing a Birth Certificate Application form which is available at www.pr.gov or www.vitalchek.com.

If you request more than one per application, each additional copy will cost $4.00. The Record Office will waive the fee for veterans and people over the age of 60.
