The Department of Investigation Fingerprint Unit provides fingerprinting, by appointment only, for staff of group day care centers which operate in commercial, non-residential space and care for children under the age of six. Prior to being fingerprinted, you must complete the Department of Health fingerprint application, which can be obtained from the day care center where you will be employed. The cost of fingerprinting is $110, payable by postal money order only.

All current and prospective employees of group day care centers must be fingerprinted to fulfill a requirement of the New York City Health Code.
